SUPPORT COORDINATOR - (SERVICES)

Your Support Coordinator will connect you with your local community and the materials supports and services you need to bring your plan to life. We will assist you to build the skills you need to strengthen your ability to understand, implement and use your plan. We are here to enable you to access the right supports to ensure the goals in your plan are achieved. A mixture of supports is used to increase your capacity to maintain/change, support relationships, resolve service delivery issues, manage service delivery tasks, live more independently and to participate independently your community.


As your support coordinator

  • find the best supports and services in your area
  • be creative with connecting you with your community
  • optimize your plan to ensure you're getting the most out of your funding

We also help you

  • to understand your NDIS plan and funding categories
  • teach you how to access your NDIS participant portal My Place
  • find and engage service providers or mainstream activities
  • assist you to negotiate with providers
  • implement your plan and work towards your goals
  • access support if you're in crisis
  • access quotes and assessments
  • assist you in planning ahead to prepare for your plan review

As your provider we will
